People who place bumper stickers all over their cars that contradict one another.

Loud eating, guy who sits next to me at work just coming into my cube and talking to me while I have headphones in.

I have had to shut my door 3 times today. It gets hot in my office with the door shut, but I have to be able to hear and not have the victim/witness hear the grab-@$$&%#

I used to work with a woman who would suck on a suckers all day. Yes, a grown woman. She would clank that SOB on her teeth constantly. That’s when I should have started therapy.

This. Also, coworkers who go off on angry, profanity-laced rants that often include yelling and sometimes involve slamming doors, slamming tables, throwing things (like a wireless keyboard earlier today), etc. This is even more distressing if there are customers down the hall at the retail counter picking up their order who can hear such tantrums.
